*The Billing Account Number if applicable. ** Required only if you are canceling a service or require IRD authorization.*** Unless otherwise agreed upon between the parties, the Service identified for cancellation will be disconnected upon the date requested. If any Service requested for cancellation will be replaced by a new 3C Media Solutions Service, please make reference of this in the notes field. 3C Media Solutions services are all scheduled based upon availability and on a first-come, first- served basis. ALL requests MUST be received at least 30 days in advance of the service date.
