Release Forms for Faculty and Student Productions (samples)

Likeness Release
For programming to be broadcast by 3C Media Solutions, one needs signed release forms from all whose faces are recognizable. In public settings people in the background do not need to sign a release.

Location Contract
In a perfect world this contract would be superfluous, however it is a suit happy society and the location contract protects both the property owner and the video production team. Getting permission is also a courtesy that can prevent you from being removed from a location. If you are shooting a production on private property of any kind, complete the Location Contract.

Programming Permission and Release Form
This form allows 3C Media Solutions to broadcast your production. Applications will not be accepted without this form.

Copyrighted Materials Permission Request Letter
The copyright permission request letter is available for faculty and students to obtain permission for the use of copyrightable material. These materials include copyrighted music, video clips, still pictures, and text.

Under Age Eighteen Release Form
This sample release form is available for students or program participants under the age of eighteen. Must be signed by a parent or guardian.
